NOTE:  If you worked on the Mayo Clinic Rochester team with RAIN in May 2018 you do not need to resubmit info, you just need an updated Flu Shot when they are available.

Thank you for joining the RAIN team supporting Mayo Clinic Go Lives.  This page will provide details on client requested On-Boarding Documentation.  Please read this very carefully and provide documents exactly as described.  Thanks in advance for your compliance.

FYI, if this info is not provided by September 20, 2018 we need to remove you from the schedule so that we are complaint with Mayo Clinic requirements.  Please take care of this process quickly.

When you have uploaded everything requested please submit the Form at the bottom of this page.

Another Go Live later this year is planned for Mayo Clinic, providing this information should complete most of the requirements for that Go Live as well, should you decide to support both of them.  Anyone on the Rochester schedule has preference for further Mayo Go Lives.

If the information you provide is not compliant with Mayo Clinic requirements, you will not be able to work onsite at Mayo Clinic.  For that reason it is extremely important that you pay attention to details.  Thanks in advance for your efforts.

Instructions for Uploading Documents to your RAIN Profile

To Upload Documents in your Profile click the following links:

• Go to My Profile
• Files & Links (on right side of screen)
• Manage Files and Links (smaller letters, towards top and left side of screen)
• Add A New Item
• Browse your computer to find files, select file and add NAME of file then submit - NOTE: The file name cannot include any punctuation or it will not open
• Check box that you agree with Upload Policy

 

RAIN Profile and Photo

Review your profile to make sure cell phone and email address are correct.  Upload a photo suitable for the badge RAIN will make for our consultants.  Badges will be distributed onsite at Mayo Clinic when you arrive at your assigned location.

A Mayo Clinic Ribbon will also be attached to your badge to ensure you are part of Go Live Support.

Mayo Clinic Dress Code

The Mayo Clinic requires a Business Professional Dress Code.

• Coat and Tie for men, dress shoes
• Suits or nice slacks and shirts/sweaters for women
• No unnatural hair colors
• Prefer no colognes or perfumes
• Mayo provides Scrubs for OR areas
• Green Vests will be provided to all Go Live Support consultants
• Mayo Clinic in Rochester is regarded as a top medical facility in the world, image and conduct must be held in highest regard for anyone onsite - thank you.
• Anyone outside of the Dress Code policy may be asked to leave

 

Patient Confidentiality

Abuse of Patient Confidentiality is terms for immediate dismissal of any RAIN Consultant.  They let us know there are triggers in their systems when any patient access is unwarranted.  Please keep this in mind and be very careful not to review info on any patients that is not absolutely necessary.  Examples given were celebrities, neighbors, or co-workers being reviewed against code.

 

Titers Note:  To comply completely with Titer Requirements you may need to receive new Titers that clearly show Immunity and also clearly show the Vendor/Clinic.  If your work situation allows you to do this somewhat quickly and cheaply we suggest considering this option.

 

Varicella Titer

Detailed description is copied below per Mayo Clinic requirements.  Make sure uploaded forms are legible and clearly show clinic, address, phone, and person who administered.  Select COMPLETE in Submission Form below after upload.  Document must satisfy ONE of the options below.

 

 • Proof of immunity via an IGG antibody titer test. (Note: equivocal is considered NOT immune) Document must show vendor's name, name of medical professional and/or clinic and, preferably, address and phone.
• OR Proof of a completed series of 2 immunizations given at least 28 days apart (reps will be considered compliant with this credential while shot series is in progress). Both dates must be clearly documented from a medical professional showing name of vendor, name of medical professional and/or clinic and, preferably, address and phone. (Zostavax, for Shingles, is not considered an immunization for Chicken Pox.)
• OR A document clearly indicating history of Chicken Pox or herpes zoster (shingles) from a medical professional showing name of vendor, name of medical professional and/or clinic and, preferably, address and phone.

 

MMR Titer

Detailed description is copied below per Mayo Clinic requirements. Make sure uploaded forms are legible and clearly show clinic, address, phone, and person who administered. Select COMPLETE in Submission Form below after upload. Document must satisfy ONE of the options below.

 

• Proof of immunity for all three diseases (Mumps, Measles, and Rubella) via an IGG antibody titer test. (Note: equivocal is considered NOT immune.) Document must show vendor's name, name of medical professional and/or clinic and, preferably, address and phone.
• OR Proof of a completed series of 2 immunizations given at least 28 days apart (member's will be considered compliant with this credential while shot series is in progress). Both dates must be clearly documented from a medical professional showing name of vendor, name of medical professional and/or clinic and, preferably, address and phone.
• OR A signed declination form is an option for those born before 1957. If you qualify and choose this option, send a ContactUs email (include your year of birth) and we will get a Declination Form to you.

 

TB Skin Test

Mayo Clinic requires that you provide a Tuberculosis Skin Test within SIX MONTHS of the Go Live.  The date of the test should be from November 1, 2017 or more recent.  Anything prior to that date is not valid.

PPD, Mantoux, TST, or Quantiferon Gold qualify.  Please ensure your documentation includes all details described below, or make arrangements for a new test that provides all requirements.

Upload a detailed and legible recent TB test result.  If your situation requires an X-Ray, please follow directions below.

 

• Read date and results of testing. (A negative result is determined by less than 10mm induration read 48-72 hours after testing.)
• The organization or health center that performed the test. Information must be on doctor/clinic letterhead (preferably also indicating address/phone) and clearly indicate member's name. Self-attested documentation will not be accepted.
• Due to occasional shortages of testing materials, we also accept negative results of Interferon Gamma Release Assay (IGRA-type) blood testing.
Note: In the case of positive test results, members will need to submit one clear x-ray result and a statement (letter or form) from their physician that they are free of TB symptoms. This letter, not the x-ray, will need to be uploaded to Profile.

 

Hepatitis B Immunity Titer Test

Please read detailed instructions below, one of the bullet points will suffice.  Upload document to your RAIN Profile and mark the Hep B question on the Submission Form at bottom of the page as COMPLETE.

• A document from a medical professional showing proof of immunity by way of a surface antibody titer test. (Note: equivocal is considered NOT immune) Document must show vendor's name, name of medical professional and/or clinic and, preferably, address and phone.
• OR a document showing dates of the series of 3 immunization injections. Note: This credential will be satisfied as rep stays on schedule to complete all 3 injections. Non-responders will need to submit their most recent series of vaccinations. All dates must be clearly documented from a medical professional showing name of vendor, name of medical professional and/or clinic and, preferably, address and phone.

Seasonal Flu Shot

Mayo Clinic requested all consultants provide proof of a seasonal Flu Shot.  Please read description below and upload to your RAIN Profile.

• A document from a medical professional or clinic with evidence of the date the rep received the current season's flu vaccination (shot or mist). Information must be on doctor/clinic letterhead (preferably also indicating address/phone). Note: Homeopathic vaccinations will be forwarded to facilities for acceptability decision.

If you have not had a Flu Shot yet, and are not opposed to getting one, please do so now to complete this requirement.  If you oppose or decline getting a Flu Shot, please send a ContactUs email via the RAIN website and we will work with Mayo Compliance to verify eligibility.

HIPAA Training Completion

Mayo Clinic requires that all healthcare professionals have received and completed training on HIPAA.

Please search your own records for any certificate of completion you may have received on HIPAA, and upload it to your RAIN Profile.  Your employer may be able to provide you with this too.  This documentation will be helpful but is not absolutely required.

When completing the Submission Form at the bottom of this webpage, mark the box for HIPAA Training as YES.  If you have never received any training in HIPAA please submit a ContactUs form stating that you are missing this requirement.

 

Criminal Background Screen

Mayo Clinic is requiring all RAIN consultants to have had a Criminal Background Screen during the previous 12 months.  If you have access to a screen that has been done for you during the previous 12 months, please upload it to your RAIN Profile.

If you have not had a Criminal Background Screen during the past 12 months instructions are below to have a new screen that will be paid by RAIN.  If you have reason to believe your background may be a problem for RAIN or the Mayo Clinic, please contact Joe Fraser ( jfraser@rainresources.com ) before initiating a new screen.

Instructions:

• RAIN utilizes Peace Of Mind (POM) for Criminal Background Screens
• Do not submit request until you have been assigned shifts on the October 2018 schedule
• Send email to Joe Fraser at jfraser@rainresources.com and provide NAME, Date of Birth, and Full Present Address, put POM Criminal Background in the Subject Line of your email.
• Include Social Security Number
